1 Because much of this information is submitted in the form of a general résumé, and because the amount of information required depends on the professional stature of the individual and the nature of his proposal, the degree of detail varies. In general, those items in category (a) represent

the minimum personal information required; those items in category (b) represent items which are often, but not always, submitted.

1 Data has been drawn from available case files, including those selected for retention for the years
1935 through 1959. Complete files for the years 1960 through 1962 are in Archives; 1963 through 1965
are in regional and main offices of the Board.

2 Includes categories of employers, unions, individuals (meaning employees, attorneys, or agents of
employers and unions).

7 Under most circumstances, these are returned to unions, but employer payroll lists are checked
indicating that the authorization cards had been received.

Requested to be submitted by employers; used to check authorization cards upon filing of petition.
A log only of callers to regional offices, whether by phone or personal visit. Retained for a 6-
month period.
10 Professional applicants for employment with the Agency.

11 Meaning without the execution of the form, case cannot be processed to conclusion.
12 Agency policy.

13 Agency policy; remain confidential unless and until witness testifies at a hearing.

14 Submission is conditional if case filed by employer.

15 List is mandatory. After election it is confidential.

16 Where challenges are determinative of outcome of election, name of each voter challenged, by whom, and reason are matters of public record.
