attendance at your assigned worksite on the following days:

A hearing regardin your termination has been scheduled for JAD - 24-1474 3.31/p.B. at Florence #firest 2307 S. Compton me. Lt. Goool. You are no longer regnized to report to any training or worksite between the time of this notification and your hearing.

You are entitled to be represented by a person of your choice at your own expense, and present witnesses on your behalf at your hearing. You may be able to obtain legal representation by contacting the Legal Aid Foundation of Los Angeles at (213) 487-3320. Should the Hearing Officer uphold the original decision to terminate, you will not be eligible for General Relief or participation in a County employment and training program for sixty (60) days from the date of infraction that caused the termination.

If you do not appear for the hearing, it will be assumed that you have agreed with the action taken, and you will not receive further notice regarding your termination.

SPECIALIZED CAREER REHABILITATION EMPLOYMENT PROGRAM

February 6, 1984

Effective February 6, 1984, distribution of the training stipends for the Los Angeles County Specialized Career Rehabilitation Program is being changed to contracted check cashing/food stamp sites throughout the Los Angeles area.

If your stipend is not at this vendor site or if your stipend... amount is incorrect, please do one of the following:

1. Telephone the SCRE? Hotline at 974-7372, 974-7373 or 974-7374.

2. Go to either the Indo-Chinese Service Centar, 124 N. Spring St., Los Angeles, CA. 30012 or the Florence-Firestone Service Center, 7807 S. Compton Ave., Los Angeles, CA. 90001 for assistance.

For further information about your stipend or the SCREP Program, please telephone the Hotline or go to one of the Service Centers listed above.

Remember that you must provide the Department of Community Services with your current address at all times. Address changes may be mailed or submitted in person to one of the Service Centers listed. FAILURE TO PROVIDE YOUR CORRECT ADDRESS WILL DELAY RECEIPT OF YOUR STIPEND AND MAY RESULT IN YOUR BEING TERMINATED FROM THE PROGRAM.

DECLARATION OF ARTHUR BARRAGAN

I, ARTHUR BARRAGAN, declare:

1.

I am 40 years old and a resident of Los Angeles County.

2. I began receiving General Relief (GR) benefits in December,

1982.

3. In September, 1983, I was referred to the new county job training program.

4. On November 3, 1983, I went to the Mexican American Opportunity Foundation (MAOF) where I was to receive job training. I picked up my check from Mr. Madrid and was told to return for training on November 7, 1983.

5. On November 7, 1983, I was in great pain in the area where I had prior hernia surgery. I called MAOF and spoke to Mr. Madrid. He told me to get medical treatment and to bring a doctor's excuse and there should be no problem. He did not say anything about any particular time to bring in the

excuse.

6. On November 7, 1983, I called two county clinics to seek medical treatment, Roybal Clinic and El Monte Clinic. Both clinics told me I could not get an appointment for a month. 7. I was able to get an appointment at the County USC Medical Center for November 15, 1983. After I went to my appointment, I took a note from the doctor to Mr. Madrid on the 15th. He accepted the doctor's statement. He did not tell me that I had missed any deadline to bring in the statement. I told him they had given me another appointment for November 17, 1983. Attached as Exhibit "A" is a copy of the doctor's statement.
