DEAR MR. CHAIRMAN: I am forwarding herewith information provided by NASA in connection with the Apollo 204 accident furnished to the Subcommittee on NASA Oversight.

This supplements information provided in extensive hearings held at your request both in Washington and in the field.

Although the one unique cause of the tragic accident which claimed the lives of Astronauts Virgil I. Grissom, Edward H. White II, and Roger B. Chaffee was not specifically identified, I believe that a proper and thorough investigation has been made and appropriate actions taken to minimize the possibility of a reoccurrence.

This information provides a comprehensive record of actions recommended and taken.
