NOTES.

1. Under the head of REMARKS, the date of any soldier's joining, whether originally or from any absence; all changes of rank by promotion, appointment, or reduction, with date of same, and No., date, &c., of order; all authorized stoppages, fines, sentences, with No., date, &c., of order, &c.; in case of ABSENCE, the nature and commencement of, No., date, &c., of order, and period assigned for same (to be repeated on every roll while it lasts); if wounded in battle, or injured on duty; if sick or confined, a remark to that effect, &c., and if confined by civil authority, nature of offense, whether discharged without trial, or if tried, whether acquitted or convicted, with dates, &c., must be carefully stated opposite to the name of the person concerned, with everything else necessary, either to account fully for every individual, to guide the paymaster, or insure justice to the soldier and to the United States.

2. In noting STOPPAGES to be made for loss or damage to public property, the gross amount due for Ordnance, Horse Equipments, &c., will be separately stated.

3. All names and dates must be written out; dotting and using the word "ditto" are prohibited.

4. Additional pay, due under Sec. 2, act of August 4, 1854, will be thus noted, viz: $2 per month for five years' continuous service, $3 per month for ten years' continuous service, $4 per month for fifteen years' continuous service, &c. That due under Secs. 1216 and 1285, Revised Statutes of June 22, 1874, thus: "For Cert. of merit, $2 per mo." If a soldier is entitled to re-enlisted pay under Sec. 3, act of May 15, 1872, it will be stated as follows: "Entitled to re-enlisted pay.' 5. The roll of those belonging to the hospital will be immediately followed by that of those who, since last muster, have ceased to belong to it. These will be classed in the following order, viz: Discharged, Transferred, Died, Deserted; and the utmost particularity will be observed in the remarks concerning them -Date and place will, in every case, be given; and No., date, &c., of orders or description of authority, be always carefully specified. When soldiers are retained in service beyond the period for which enlisted, as shown by the date of their enlistment, the cause of such retention should be stated. Soldiers discharged and re-enlisted, or who have deserted and been retaken, since last muster, should be placed both in the body of the roll and under the appropriate headings as having been discharged, deserted, &c. In cases of desertion, whether apprehended or surrendered, with dates and place, will be stated.

NOTES.

6. The remark "discharge and final statements given" will be made opposite the name of every discharged soldier to whom such papers have actually been given. Blank spaces under the head of LAST PAID are to be filled as usual. The character given on the discharge will be noted on the rolls.

7. Within three days after each regular muster, the mustering officer or commandant of the post will transmit to the Adjutant General a copy of the muster-roll of each hospital. Blanks will be supplied from the Adjutant General's Office, and will be accounted for on the muster-rolls.

8. The name of every soldier in hospital, whether with or without a descriptive list, should appear on this muster-roll, and every one on the last muster-roll of the hospital must be accounted for on subsequent roll.

9 Hereafter, ruled blanks will not be used; but when one roll will not contain all the names, the inmates of the hospital will be mustered by wards, and separate rolls made out for each ward; the whole will then be tied together at the top. 10--Citizen physicians will appear on the roll next below the commissioned officers. 11. In cases of sickness, injury, or wounds, the nature of same, whether contracted in line of duty, and if wounded or injured, location, &c., should be carefully stated. 12. Four muster and pay rolls are made at each muster; one to be retained with the hospital records, and two for the paymaster, and one with the pay-roll portion cut off to be forwarded to the Adjutant General by MAIL direct.

I certify, on honor, that this Muster-Roll is made out in the manner required by the printed notes; that it exhibits the true state of the Hospital and General Non-Commissioned Staff, U. S. A., for the period herein mentioned; that the "Remarks" set opposite each name are accurate and just; and that the "Recapitulation" exhibits in every particular the true state of the hospital, and attached, as required by the Regulations and Rules and Articles of War.
